Information on the processing of your personal data

From May 25, 2018, the Regulations of the European Parliament and of the Council (EU) 2016/679 of April 27, 2016 apply. on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (General Data Protection Regulation – GDPR) (Official Journal of the European Union of May 14, 2016 L 119/1 ). One of the assumptions of the GDPR is to provide customers with detailed information regarding the processing of their personal data, so please read the following information:

  1. The administrator of your personal data is ALL4MED SP. Z O.O. creating BLUE HEALTH ZONES. The administrator uses all the necessary organizational and technical measures to ensure the confidentiality, integrity and availability of your data.
  2. Your personal data will be processed in order to provide the services offered in the rehabilitation office pursuant to art. 6 sec. 1 letter c of GDPR in connection with the act of April 15, 2011 on medical activity, and the act of November 6, 2008 on patient’s rights and the Patient’s Rights Ombudsman. Personal data in the form of a telephone number and e-mail address – will be processed for the purpose of: – telephone contact aimed at arranging and changing the date of the visit or sending an SMS notification about the upcoming visit,
  3. – direct marketing of the services offered,
  4. – sending a newsletter or information about current promotions, based on your consent in accordance with art. 6 sec. 1 lit. a GDPR
  5. Your data will not be transferred to other recipients or to a third country or an international organization.
  6. Your personal data will be stored for the period specified by law – for a period of 20 years counted from the end of the calendar year in which the last entry in your medical records was made
  7. You have the right to request the administrator to access your personal data, rectify it, limit processing, and in relation to data processed on the basis of consent, the right to delete data and the right to withdraw consent to the processing of personal data at any time without affecting the lawfulness of processing, that took place before the consent was withdrawn.
  8. You have the right to lodge a complaint with the supervisory body dealing with the protection of personal data, which is the President of Personal Data Protection (00-193 Warsaw, Stawki 2), if you believe that the processing of your personal data violates the provisions of the regulation indicated at the beginning.
  9. Providing your personal data is a statutory requirement. Providing a telephone number and e-mail address is voluntary, dependent only on your consent, but the consequence of not providing them will be the inability to quickly communicate with you, e.g. to change the date of the visit, no possibility of direct marketing of the services offered, no possibility of sending a newsletter or information about current promotions .
  10. Decisions made with regard to your personal data will not be made in an automated manner, and we will not apply profiling to them.
